Job Description Job Description Job Summary: The EIS Case Manager will play a pivotal role in providing comprehensive case management services to clients newly diagnosed with HIV/AIDS or those returning to medical care, ensuring the collection of all necessary eligibility documents from eligible clients. Empower individuals living with HIV/AIDS and LGBTQIA community members to navigate the healthcare system and access crucial services through holistic case management. Duties & Responsibilities: Responsible for medication assistance program applications and recertifications. Provides case management to all clients eligible for Early Intervention Services. Benefits counseling by assisting eligible clients in obtaining access to other public and private programs for which they may be eligible. Completes client updates and intakes for clients eligible for AARC/ASCEND services. Maintains regular follow-up and completes needs assessments and care plans with clients, with a goal of client self-sufficiency. Coordinates and monitors medical appointments, medical adherence, and Medication adherence. Facilitates Health Education Risk Reduction for clients living with HIV/AIDS. Coordinates medical care, follows up on treatments, and monitors treatment adherence. Initial assessment of case coordination service needs Development of a comprehensive, individualized care plan Timely and coordinated access to medically appropriate levels of health and support services and continuity of care. Provide eligibility updates for EIS emergency walk-ins or Scheduled EIS updates. Other administrative duties assigned by the Director of Case Management Perform other duties as assigned. Qualifications & Skills: Proficiency in assessment of needs and formulation of care plans. Excellent written and communication skills. In-depth knowledge of community resources and services. Data Entry and system navigation skills Strong written and communication skills Knowledge of community resources and services Provide unbiased, competent, and ethical services. Bilingual in English and Spanish. Education, Licensure & Experience: A minimum of an Associate’s degree from an accredited college preferred, but not required. A minimum of a Bachelor’s degree preferred, but not required. Minimum of one year of paid work experience with individuals living with HIV/AIDS Work Schedule: Regular schedule Monday to Friday 8AM - 4:30PM Flexible schedule, if required Work Location: One location, In person Off-site, if required Benefits: Employer 401(k) retirement plan match Health insurance Dental and vision Disability insurance Life insurance & AD&D Employee assistance program Paid time off Paid holidays Requirements: Must maintain a valid driver’s license and current automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive company owned vehicle or your personal vehicle during company business. While performing the duties of this job in a traditional indoor office, the team member is exposed to a medical office environment having direct/indirect contact with patients and clients. Team member is also exposed to a social service work environment. Some job duties may include travel and outdoor environment. Physical demands refer to the level and duration of physical exertion generally required to perform tasks in support of job functions, for example - sitting, keyboarding, walking, lifting, carrying, reaching, pushing, pulling, bending, stooping, twisting, turning, climbing step stools, and standing for periods of time. Must be able to lift and maneuver a minimum of 15 pounds using proper safe lifting techniques.
