As a Care Manager, you will be responsible for delivering personalized care and support, ensuring the comfort and well-being of individuals in need. Key Responsibilities: - Assist clients with daily living activities, including bathing, dressing, grooming, and eating. - Provide companionship and engage in meaningful conversations to enhance the client's quality of life. - Monitor and record changes in the client's physical and emotional health, and report any significant observations to the appropriate personnel. - Help maintain a safe and clean environment by performing light housekeeping and laundry duties. - Prepare and serve meals according to dietary needs and preferences. - Assist with medication reminders and ensure adherence to prescribed healthcare regimens. - Accompany clients to medical appointments, errands, and social activities as required. - Foster a positive and supportive atmosphere that encourages client independence and dignity. Qualifications: - Previous experience in a caregiving or similar role is preferred. - Basic knowledge of home care practices and techniques. - Ability to communicate effectively and respect client privacy and confidentiality. - Patience, empathy, and excellent interpersonal skills. - Certification in First Aid and CPR is an advantage. - Flexibility to work various shifts, including evenings and weekends.
