Job Description Job Description Brief Job description: Quality
of Life Holistic Care PLLC -We focus on medical, mental, and
spiritual health. 1. Primary care in senior living locations. Small
clinic space in 4 locations, looking to add multiple others. You
would coordinate with my two care coordinators Help oversee CCM and
RPM programs Help oversee CoCM and coordinate with LPC/psychology
for psyche Monitor prayer room 2. Wound Care Standard wound care
Ultramist wound care Wound care with graft 3. Hospice F2F 160 pts
on my hospice panel, you do f2f (I will train as needed) 10-15
visits per week = 1500 to 2000$ per week 4. Coordination with home
health -I partner with 4 HH at the moment, looking to add a 5th.
Equals 700 pts -some PCP, this is mostly for wound care patients 5.
Rounding on our patients at PAM IRF with Dr. -can round daily.
Requires credentialling with PAM. 5. Wellness Clinic: coming soon
2025 Fall 6. Many new business opportunities on the way.
Developmental opportunities. -Interested in flexible NPs who are
team oriented. Bonuses and many incentives. Business is start up
and open for -I do many marketing events. NP needs to enjoy meeting
people and selling our products (PCP, WC, and other
