Job Description:
Job Description Job Description The LVN will provide direct
nursing services for adults with severe and persistent mental
illness, assist with medical treatments, complete documentation for
the clients. Provide nursing services to clients to include
medication injections, assist physicians and team with caseloads.
Educate clients on medications, and general health issues. Along
with psychiatrist and Nurse Practitioner, nurse serves as the
medical specialist on ACT team. Assist with medical
treatments/interventions appropriately, taking initial health
history on new patients and conducting a monthly or quarterly for
all clients on their assigned team. Transport consumers to sites of
medical interest (as needed) – This position is field-based
nursing, employee will provide visits to consumers in their homes
