Job Description:
AccentCare is seeking a dedicated and compassionate RN / Patient
Care Manager for our Home Health services. In this role, you will
oversee a team of nursing and allied health professionals to
deliver exceptional care to patients in their homes, ensuring both
the clinical and emotional well-being of our clients. Your
expertise will guide your team in providing care according to the
latest health guidelines and our own high standards. This position
involves managing care plans, coordinating with physicians, and
supporting patients and their families. Oversee patient care
services and ensure compliance with healthcare regulations and
AccentCare policies. Develop and review care plans in collaboration
with physicians and healthcare team members. Monitor patient
progress and adjust care plans as necessary. Manage and train a
team of healthcare professionals. Coordinate with other healthcare
providers and family members to provide holistic care. Ensure
documentation and care procedures are completed accurately and on
time. Strong leadership and team management skills Excellent
communication and interpersonal skills In-depth knowledge of
nursing practices and home health regulations Ability to handle
stressful situations with compassion and patience Proficient in
healthcare IT systems and patient documentation Medical Insurance
Dental Insurance Vision Insurance 401k Current registered nurse
(RN) license in the state of practice CPR certified Preferred:
Certification in home health care or health care administration
