Job Description:

inpatient rehabilitation, home health, and hospice, is seeking a
compassionate and skilled Licensed Practical Nurse to provide
high-quality care in a dynamic healthcare setting. This role
demands a dedicated individual eager to contribute both
independently and as a part of a team dedicated to excellence in
care. Our ideal candidate will administer nursing care in a
comfortable and therapeutic environment, focusing on safety,
treatment, and recovery. Administer basic patient care, including
medication, wound dressing, and vital sign monitoring. Collaborate
with registered nurses and doctors to plan and evaluate the quality
of care. Record patients' medical histories and monitor changes in
their condition. Educate patients and their families on treatment
plans at home and lifestyle changes post-discharge. Assist in
testing and follow-up appointments to ensure continuity of care.
Strong interpersonal communication skills Critical thinking and
decision-making abilities Comprehensive knowledge of hygiene and
healthcare protocols Efficiency in electronic health record (EHR)
software and data entry Medical Insurance Dental Insurance Vision
Insurance 401k Valid Licensed Practical Nurse (LPN) license in the
state of practice CPR and Basic Life Support (BLS) certifications
preferred
