Job Description:

Touchstone Communities, a leader in senior living and long-term care, is seeking a dedicated MDS Coordinator to join our team in Texas. This role is pivotal in ensuring that our care standards are met through the accurate and timely completion of the Minimum Data Set (MDS) assessments. The right candidate will assist in optimizing the quality of care and enhancing the living experience of our residents by focusing on individualized care planning and facilitating comprehensive assessments. Coordinate and conduct comprehensive and systematic MDS assessments of each resident. Ensure timely submission of accurate assessments as required by state and federal regulations. Collaborate with healthcare staff to develop and review individualized care plans based on MDS outcomes. Monitor and document the residents’ responses to treatment and modify care plans as needed. Provide training and support to staff regarding MDS process updates and compliance requirements. Participate in quality assurance procedures to continually improve the standard of care provided. Strong knowledge of MDS 3.0 and the RAI process. In-depth understanding of medical terminology and healthcare operations. Proficiency in electronic health record (EHR) systems. Excellent observational and analytical skills. Effective communication and interpersonal skills. 401k Registered Nurse (RN) licensed in the state of Texas. Certified MDS Coordinator (CMDS) credential is preferred.
