Job Description:

detail-oriented Wound Care - Treatment Nurse who is dedicated to
providing exceptional wound care and promoting skin integrity for
our residents. This is more than a clinical role - it's an
opportunity to build meaningful connections and play a vital role
in improving the quality of life for those we serve. Key
Responsibilities: Deliver individualized treatment and wound care
in alignment with physician orders and established care plans.
Assess, monitor, and document residents' skin integrity,
collaborating with the nursing and interdisciplinary teams to
ensure optimal healing outcomes. Partner with physicians and
licensed nurses to develop and modify treatment regimens that
address both the physical and psychosocial needs of residents.
Educate residents, families, and staff on skin integrity, wound
prevention, and care best practices. Maintain compliance with all
applicable federal, state, and local regulations, as well as
company policies and procedures. Support a positive,
teamwork-driven environment that upholds Touchstone's mission and
values. Qualifications: Current and valid Texas LVN license , or a
Compact State RN/LVN license in good standing. Prior experience in
skilled nursing, wound care, or long-term care preferred. Strong
clinical judgment and attention to detail. Excellent communication
