Job Description & Requirements - Specialty: Care Manager - Discipline: RN - Start Date: 06/09/2025 - Duration: 52 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Local Contract Job Summary As supervisor, oversee the team daily. Focus on evaluating and coordinating patient care. Ensure optimal health outcomes by managing resources effectively. Deliver quality care that meets patient needs. Location: Onsite at Austin and Buda Facilities Setting: Case Management Schedule: FT Monday-Friday Essential Functions of the Role Supervising your assigned staff both administratively and clinically, according to established industry best practices. Assisting and training your staff in identifying the needs of patients and their families, thereby ensuring top-notch care. Playing an effective role in crafting and overseeing care plans, while coordinating and delivering services. Instituting practice and care standards that contribute to positive clinical outcomes, financial stability, and patient satisfaction. Overseeing the development of discharge plans and strategies to meet patients' needs post-discharge seamlessly. Making sure staff follows rules and regulations for their job and institution.
