candidates are mature professionals capable of managing complex
medical and psychosocial situations. We are looking for creative

to meet the evolving needs of our patients and their families.Your
RoleAs an Advanced Practice Provider in our Palliative Care Team,
you will:
- Conduct comprehensive physical and psychosocial assessments of
patients.
- Create and implement evidence-based, patient-centered care
plans.
- Provide guidance and education to patients and their families
on disease management, symptom control, and navigating care
transitions.
- Collaborate with a multidisciplinary team, including
physicians, nurses, social workers, and chaplains, to ensure
seamless, compassionate care.
- Offer expertise in managing symptoms such as pain,
breathlessness, nausea, fatigue, and more.
- Serve as a key resource for patients and referring providers,
ensuring continuity and quality of care across settings.This
position primarily involves Monday-Friday, 8 AM-5 PM clinic care,
giving you the opportunity to provide deeply impactful care while
maintaining work-life balance.About YouWe are looking for a team
player with excellent communication skills and a passion for
delivering patient-focused care. The ideal candidate will:
- Demonstrate a commitment to holistic care and a genuine
interest in supporting patients with serious illnesses.
- Be skilled in critical thinking, complex problem-solving, and
evidence-based decision-making.
- Have experience in or a strong interest in palliative care, or
related specialties.
- Be self-aware and have the capacity for introspectionRequired
Qualifications
- Education: Graduate of an accredited Nurse Practitioner or
Physician Assistant program.
- Licensure: Current license to practice as an NP or PA in
Texas.
- Certification: Current certification by the appropriate board
(NCCPA for PAs; AANP/ANCC for NPs).
- Basic Life Support (BLS): Certification within 30 days of hire
or transfer.
- Experience: We would prefer someone with some medical
experience, but are open to motivated new gradsKey Skills And
