work within our Skilled Nursing and Rehab Community. As a

for career growth. We are currently offering a Sign-On Bonus of
$3,500 for this full-time 2p-10p RN position ESSENTIAL FUNCTIONS:
The following is not an exclusive list of responsibilities. The
list of essential duties and responsibilities is intended to
provide a representative summary of the major duties and
responsibilities performed. Incumbents may be required to perform
other job-related tasks other than those specifically presented in
this job description. Assist in implementing resident care policies
and procedures. Assess, evaluate, plan, and implement resident care
plans and respond to resident needs. Ensure communication with
family in the event of patient transfer to hospital or post-acute
facility. Ensure appropriate documentation of medication
discrepancies. Chart Compliance Reviews. Ensure monthly
documentation and review of risk indicator reports with specific
areas of infection control, incidents, skin concerns and medication
errors. Check rooms routinely to insure residents are being
provided the care requested. Delegate responsibility as needed to
nursing personnel. Maintain charts, make observations. Order
supplies and equipment, carry out instructions of supervisors, and
observe departmental safety standards and regulations. Able to
recognize significant changes in the conditions of residents and
take necessary action. Supervise direct resident care personnel in
the unit. Ensure that the individual nursing care plan is followed.
Administer or supervise the preparation and administration of
prescribed medications. Administer or supervise prescribed
treatments. Supervise serving of prescribed diets and fluid intake
(it is acceptable to document only deviations from normal) and
report persistent unresolved problems to the physician and the
Director of Nursing. Ensure that all medications and treatments
charted appropriately by the person administering the medication or
completing the treatment on that person---s assigned shift.
Supervise the preparation of incident and accident reports. Approve
and sign each chart when observations are made. Each chart must be
signed in accordance with State standards. Ensure that a copy of
the medical record or an abstract of it is forwarded with the
transfer of resident. Ensure that drugs covered by the Controlled
Substances Act are verifiable by inventory. Assure appropriate
destruction of medications. Participate in regular staff meetings.
Responsible for the detection and correction of situations that
have a high probability of causing accidents or injuries to
residents. Provide nursing care relevant to condition and
prescribed care as frequently as necessary to maintain quality of
resident care. May perform minor housekeeping duties around the
nursing unit. Perform routine charting. Assist aides and orderlies
as necessary. Instruct resident in self-help and self-care. Other
duties as assigned by the Assistant Director of Nursing or Director
of Nursing. GENERAL FUNCTIONS: Maintain confidentiality of The
Wesleyan and facility information. Follow Client---s Rights
policies at all times maintaining privacy, confidentiality and
respect. Ability to understand and comply with applicable Federal,
State and local regulations as they apply to this job. Attend all
required department and/or facility meetings. Adhere to applicable
laws, rules, regulations and Code of Conduct directed by Corporate
Compliance Program, conduct business using ethical standards, and
report any suspect or actual event or circumstance to the
Compliance Officer. Understand and comply with use and disclosure
rules of resident Protected Health Information (PHI) as required by
the HIPAA Privacy Rule of the Health Insurance Portability and
Accountability Act. Demonstrates leadership and problem-solving
skills. Supports mission statement, philosophies, and objectives
promoting safe delivery of home health/hospice/PAS services.
Reports to work promptly as scheduled without absences. Maintains a
professional appearance and a business-like attitude. Functions
within the job description of his/her position. Complies with
agency policies set forth in the Code of Conduct and Employee
Handbook. Work cooperatively with all community members and
willingly accept assigned duties. Report any possible problem or
concern regarding community members to Assistant Director of
Nursing /Director of Nursing/Administrator immediately. 15. Perform
any other related duties as directed by Assistant Director of
Nursing/Director of Nursing /Administrator. community members
(staff, clients, families, and vendors) QUALIFICATIONS: Must be a
registered nurse in the State of Texas with a current license. One
year of nursing experience in geriatrics preferred. Must possess
leadership ability and the willingness to cooperate with other
department heads. Ability to organize, plan and direct nursing
service. Must handle emergencies, give emergency care and give
emergency instructions. Must possess proficient computer skills.
Ability to maintain confidentiality in all resident and staff
matters. Schedule: 1:45 p.m. --- 10:15 p.m. The Wesleyan---s
