Summary and Qualifications Job Summary The Disease Management
Coordinator provides leadership and promotes excellence in patient
care of cardiac, neurologic and other disease conditions in the
Emergency Department, ICU, Medical Surgical and Rehabilitation
Units, along with other entities within the healthcare team. This
position is responsible for planning, development and
implementation of program goals and operations including Chest Pain
Accreditation by the Society of Cardiac Patient Care, Primary
Stroke Center Designation and Trauma Designation. The Disease
Management Coordinator also coordinates education of staff and
physicians, oversees the ongoing quality improvement process to
improve patient care. This position ensures compliance with the
accreditation standards and collects and reports data to external
agencies (as required) and internal committees. --- Performs daily
concurrent chart review to ensure compliance with evidence based
program measures for Stroke and AMI. --- Retrospectively abstract
Stroke and AMI data from the medical record (HPF, Meditech, etc.)
and enter data into the appropriate vendor database (Comet, NCDR,
etc). --- Perform Inter-Rater Reliability chart review. --- Ensures
all data reporting timelines are met (IRR, normalization, etc). ---
Collaborate with clinical staff, physicians, and leaders to ensure
evidence based measures are met in a timely manner. --- Implements
and revises the required policies, procedures and protocols for
Trauma Services, Stroke Services and Cardiac Care. --- Initiate
patient transitions thru departments, and movement of patients to
other levels of care; i.e. rehab (as needed). --- Perform
patient/family education on Stroke, Cardiac Care and Trauma
prevention (as needed). --- Participates in trauma service data
collection and data analysis. --- Assists the Trauma Medical
Director (TMD) and Director of Emergency Services in ensuring the

Verification/ Designation as a Level IV Trauma Center. Requirements
Experience Required: 3 years as an ER or ICU nurse Required:
Graduate of accredited nursing school, Associates Degree or higher
lever education. Preferred: Experience as a staff nurse in an acute
care setting License and Certifications LICENSES AND CERTIFICATES:
Required: Texas RN license; BLS and ACLS/ALS certification required
as per St. David's HealthCare policy.
