The RN Case Manager will be responsible for planning, organizing and coordinating client care responsibly for all assigned cases, with an emphasis on community health education/experience. Utilizing resources of the community to develop, implement, and evaluate individual client care plans and update as needed according to established professional standards of care and serving as a liaison between the agency and the client or client's family in order to meet their needs within their home or communities are essential to the success of this position. As a RN Case Manager at The Wesleyan, you will be an integral part of a mission and values-based organization that provides endless opportunities for career growth.Essential Job Functions:Completes an initial assessment (OASIS) of patients to determine home care needs. Provide a complete physical assessment and history of current and previous illnesses.Regularly re-evaluate client nursing needs.Initiates the plan of care and make necessary revisions as client status and needs change.Uses health assessment data to determine nursing diagnosis.Process incoming referrals while coordinating with patient, family, clinical staff, scheduler and DCS.Assist with scheduling SOC, RCT, Aide supervision, ROC, Discharge and therapy Evaluations.Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventative, and rehabilitative nursing actions. Includes client and family in the planning process.Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.Review clinical notes, physician notes, orders and all other pertinent clinical documentation of the team before review of QAPI and Coding.Regularly evaluates the client's progress, in collaboration with team members; revises client's plan of care accordingly.Collaborate with DCS to organize case conference meetings.Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.Complete weekly chart audits.Collaborates with QAPI for quarterly compliance.Process labs and physician requests and communications with clinical staff in a timely manner and document communication.Acts as DCS under the Authority of the Executive Director in the absence of the DCS while maintaining Case Manager responsibilities.Rotates weekend, night, and holiday call with staff to facilitate adequate coverage for all clients.Understands and observes infection control policies and procedures through proper handling of materials.Obtains eight (8) contact hours on an annual basis of clinical in-service or training in specialty area.Responsible for the safety of patients under his/her care.Qualifications & Experience:A minimum of a college degree is requiredMust have unexpired Registered Nurse license issued by the State of Texas.Prefer a minimum of three (3) years' experience as a registered nurse, including at least one (1) year of experience in home health care.MUST have OASIS experience for all time-points: SOC (Start of Care) RCT (Recertifications) ROC (Resumption of Care) and D/C (Discharge).A "working knowledge of"-- - CMS - Medicare Guidelines for home health.Must have a current Cardio Pulmonary Resuscitation Card (CPR) issued by recognized provider such as American Heart Association or American Red Cross that includes hands-on practice and in-person skills assessment.Must have previous work experience in an office environment gathering information, preparing documents, and managing records in an accurate and efficient manner.Must have intermediate working knowledge of Microsoft Products such as Outlook, Windows, Excel and Word.A service-minded approach to all aspects of the jobAlignment with and enthusiasm about The Wesleyan's culture and core valuesMust have current physical examination showing freedom from certain types of communicable diseases.Must have current Texas driver's license and auto insurance in accordance with state requirements.The Wesleyan is an Equal Opportunity EmployerBenefits:Health InsuranceDental InsuranceVision Insurance401(k)Employee Assistance ProgramPaid Time OffTuition ReimbursementShort-Term DisabilityCompany paid Life InsuranceCompany paid AD & DCompany paid Long-Term Disability
